Dixie Baseball was needing an online registration solution for franchise rosters. They gave me a copy of the .pdf they were e-mailing to coaches and asked me to replicate the layout and design of the .pdf via a web browser. Upon a digital signature and submission of the form, the coach would be automatically transferred to PayPal where they would complete the financial part of the transaction. Upon successful payment, the coach would be returned to the original form, but now marked as "Receipt." The form contents, along with PayPal transaction information, was stored in a CSV file for Dixie to download at their disclosure.

The web application was developed in PHP on a Linux hosting environment.
